1st Trimester

No Fetal Pole

I went to the doctor at 5 weeks 6 days because of cramping. They did an u/s and found a yolk sac(measuring right at 6 weeks), but no fetal pole. She ordered a blood test for Hcg levels. My progeterone level was 23 and my first hcg level was 25, 685 (Tueday at 11:30 AM). I did a repeat test on Thursday at 8:00 AM (little less than 48 hours). My hcg levels were 34,937, so they did not double. I go back on Monday for another u/s at 6 weeks 5 days.

Is is likely that I will miscarriage or is it common to see no fetal pole?

  • I don't have any advice or anything but wanted to wish you luck.

  • I think once beta numbers reach a certain point, they don't double, but will continue to rise.

It is strange that your numbers are so high but there isn't much to see on the U/S.

It's possible you're not as far along as you think, though I don't know why your numbers would be so high if that's the case. Fingers crossed everything is okay!

  • I just stopped taking the pill on December 10 and my LMP was December 14. I didn't even think it was possible to get pregnant that fast and have no clue when I would typically ovulate. However, during the u/s the sac was measuring at 6 weeks. Are u/s always correct? My doctor said she couldn't tell me either way...said it was 50/50. She did not seem very hopeful though.

Can my numbers continue to increase even if my body is preparing for a miscarriage. Could this be a blighted ovum? Or is there another reason why I would have high numbers, but not see a fetal pole?

  • I really wish I had some advice but I'm sending T&P your way! Please keep us posted!

  • Was it an abdominal or transvaginal ultrasound? If you're bigger, abdominal scans might not work well early on--that's my experience. At 5w6d NOTHING could be seen abdominally on me, but transvaginally we saw the yolk sac and pole. At 7w1d we could only see the gestational sac abdominally but a transvaginal scan showed us the baby and heartbeat.

    Good luck, keep us updated!

  • I had an U/S at 5w5d and all they could see were gestational and yolk sacs.  I went back at 7 weeks and there was a healthy little peanut!

I hope this is the case for you, best of luck.

  • Thanks for the support.

Yes, it was a transvaginal u/s. I am average weight for my height.

I've been reading a lot of posts similar to mine and the results seem to be 50/50. I did read that with my hcg levels so high, that there should have been a fetal pole.

Sweets11 - Were your hcg levels high too at the time of your 5 wk u/s?

  • I went through this *exact* same thing!  I had spotting and red blood, so they did blood work and an ultrasound at 5 weeks, 6 days.  They found a yolk sac, but no fetal pole (and my HCG numbers were almost exactly the same as yours).  They told me to wait until the next appointment 2 weeks later for another ultrasound... and, sure enough, at my 8 week appointment, there was a baby with a strong heartbeat that we could both hear and see (and the baby measured 1 day further along than we had thought based on my LMP).  I know it can go differently for everyone, but I know what a terrible time those 2 weeks of waiting were for me... hold on to hope!  My T & P go out to you!

  • Although easier said than done, try to stay positive.  Since you JUST came off the pill, you could have ovulated at any time -- your body wasn't regular.  Hang in there and wait until your next u/s before you worry.

P.S. -- I've said this a millon times before but i REALLY hate when Dr's do these early u/s becuase things are so little at that point.  At 6 weeks, the baby has only been developing for 3 in actuality.  This happened to my sis and they were calling her a "threatened abortion" becuase they didn't see a heart beat at 5 weeks...meanwhile she is now 33 wks pregnant with my nephew.

  • It's common not to see the fetal pole that early so please don't be nervous!  My doc won't even do an u/s until after 7w for that very reason.

When do you go back for another u/s?  A week makes a HUGE difference this early on.

  • I'm going to hope for the best. Follow-up is tomorrow morning (6w5d). I will let everyone know the result.

  • At 5w5d, we saw a gest sac, a yolk sac, but no fetal pole with DD.

The next week we saw her heart beating. Good luck to you!
