I would feign ignorance and say something like, "oh my gosh, did your 5-point harness break?!" and then use that as a launching point for discussing that you have to use the 5-pt harness as intended.
eek! I think I'd just flat out say something like "are you sure your seat is installed correctly. The 5-point harness needs to be the only 'belt' touching her (and make sure those shoulder straps are over her shoulders!). The car's seatbelt goes through the holes in the side, so she shouldn't be laying on it."
Just send them a message on FB letting them know just in case they didn't know. A couple nesties have pointed it out to me when I take pics of Nico in the carseat. Once he had taken them off and another time they were too loose. I was not insulted and appreciated someone brought it to my attention and now am much more careful.
I think this goes way beyond having the straps too lose. Maybe I'm seeing it wrong, but it looks to me like they are using the car's seatbelt instead of the 5 point harness? Like, there's no seat belt holding the carseat in place? Oh my. I'm usually a fan of minding my own business, but this looks super dangerous.

I have sent many messages on Facebook regarding carseat pictures. I do what they call in management "the compliment sandwich". Info goes postive-negative-positive. "I know how much you love your kid and I can't believe how much they've grown BUT I couldn't help notice XYZ, which is not safe" Give some benefit of the doubt like mentioning that you realize it may have been a picture taken in the driveway etc). Offer to help personally if you can. End with "I think you're an awesome mom and I know you'd do anything to keep your baby safe" kind of message.
I've yet to have a negative response. I think most people just don't know. DH has started fixing his co-workers car seats when he sees how loose they are. He then schools them on how tight the harness should be. I'm so proud!!!
And if you can't find the words, send them the you tube video "The Importance of a 5 point harness" because that one movie will keep both my kids in carseats until middle school.
Thanks again ladies, I just got an message back from her and she said they just got home but LO has figured out how to unbuckle the chest clip and will be buying a new one this weekend. she was not offened at all.
