High-Risk Pregnancy

XP: Now I'm nervous...lets talk about cervical length....

I had my second u/s for cervical length check today with maternal fetal medicine.  Last time (2 weeks ago) my length was 3.5cm (thought it was 4.2?), now it's at 2.9cm.  They said they won't worry until it gets to about 2.5cm...Should I be concerned????  This seems to be a significant size change over a 2 weeks time period...previous LO born 30wk 1 day, LEEP procedure performed in August of 2011...

Thoughts please!!!!

  • In the 11 weeks since my IC diagnosis, I have seen all sorts of things on this board!  Mine was 2cm, and has stayed there.  I've seen many girls with dynamic cervix measurements, that change both up and down as the weeks pass.  I've read about many who have had a nice increase in length from bedrest and/or progesterone and/or just time passing.  At the end of the day, I trust my doctors and if they are not worried, then I'm not going to worry (too much).

    Good luck to you!!

  • It sounds like it is decreasing, and I would seek a second opinion from an MFM.  I think most are concerned when you drop between 2.5 - but you seem to be downward trending and the earlier a cerclage is placed (if that is an option), the better.

  • It was MFM that I saw today...she isn't concerned yet, but if I go in two weeks from now and it has dropped again, then they will be concerned.  I haven't talked to my regular OB about it yet.  I was supposed to have an appointment tomorrow with her but it had to be rescheduled due to her having surgery.  We talked about the cerclage...MFM said 2-1.5 or less and I would get one if I am less than 23 weeks.  I'm just really worried.

With Cameron my cervix was 3.4 at 27 weeks (right after PTL started), and then by 29 weeks my water broke (cervical length the day my water broke was 1.8cm).  I haven't even considered the idea that this LO might be born even earlier and that has me in a bit of a panicky state right now. We were so blessed that Cameron had no issues during his NICU stay, he just needed to grow.  I can't imagine how hard things will be if it goes the other direction.  Things happened so quickly with him last time, I know I won't stop worrying until I see MFM in two weeks and see that I haven't changed or the length increased.  I have had 3 progesterone shots so far...

  • Hi. I too have a short cervix, mine was down to a .8. I had to get a cerclage at 22 weeks. since then I have stayed the same and take the shots too. After the cerclage, I was getting weekly check-ups because my doctor said a lot can happen each week. I have been on strict bedrest for 10 weeks now. I know every person is different and so are the doctors; which is why you just have to go with your gut.

If I were you, I would not wait 2 weeks to go back to the doctor since you are showing signs of a shortening cervix. See if you can get an appointment scheduled after just one week - I would actually demand it. A lot can happen in 2 weeks. I would think you are already high-risk considering you had a premie. You want to take all precautions to make sure you don't have another premie.

    Best of luck!

  • Like PP said, I not sure I would wait 2 weeks and probably want a second opinion.  I was surprised your MFM wouldn't recommend a cerclage with your already dynamic cervix, previous PTL and recently having a LEEP procedure.  Have you consulted your regular OB about any of this?  My reg OB placed the cerclage and has been IMO more pro-active than my MFM.

  • We talking about a difference of less than a 1/2 cm and if you had a different u/s tech, where they start and stop measuring could be different. My cervical length actually went up over 4 weeks.

I think I replied to a previous posting you had that I'd explore a cerclage. You don't even want to be on bedrest if you have a toddler running around! I know it's risky at this point, but you still have a pretty long cervix.
