Maternity Leave Transition Plan

I need to provide a plan to my supervisor soon. I was wondering if anyone used a template of sorts? I am not sure if I should just list out my roles and responsibilities (which my boss knows) or outline daily tasks, regular standing meetings and outstanding/current projects. Any help would be great! Thanks Ladies

good idea - my plan was to flip out once my water broke and write frantic e-mails to coworkers and clients before heading to the hospital
