Nuchal translucency scan is not right?

They left a message telling me that the scan was normal. Then my ob calls me two days ago and tells that the numbers did not look right to her. Baby A measured at 1.0 and baby B measured at 2.3. She said that baby A has zero chance of having downs while baby B has a 1 in 200 chance. She wants me to see mfm right away to get a level two ultrasound. I'm really not sure how to feel about this. She seems concerned but she is also very catious about everything.Then after the level two us she said DH and I can decide if we want a amnio if the us isn't right. It confuses me because her partner thinks it was normal. Anyone else gone through this?

  • 2.3 isn't even high!  I think anything over 3 is abnormal, and if you want to be really conservative, anything over 2.5mm may be slightly abnormal.

     

    and 1/200 isn't much, that means you baby has a 0.5% of Downs.  She's also wrong about baby A.  My OB told me that a baby can have perfect scans and still have downs (very unlikely, but still possible).  The only way to definitively know is through an amnio.

  • i thought it was anything over 3...but i didnt do that test this time around with this pregnancy, so i dont remember everything. 

    i would like to say she is just being really cautious, which is good.  i would like to say to not to worry a ton about this, she is just being cautious and wants to double check everything.  when these initial tests cause some alarm, many times, further testing shows everything is okay. i would try not to read too TOO much into it. 

    if it were me, if course i would be nervous/scared.  but try to not freak out until you know the whole picture. 

    unfortunatey with these tests, they are good guidelines but they are never 100% right.  one of my friends had the lowest possible chances for Downs based on these tests, and her baby ended up having Downs.  of course that situation is rare, but it can happen.  i also know a couple people who had similar situations like yours, and everything turned out to be fine.

  • First of all, I can't believe that she would say the baby with the 1 mm NT has a zero chance of Down syndrome because that is not true. More like a 1/15,000 chance. My twins are very similar to yours, one had a 1 mm NT scan and the other had a 2.6 mm. I was told that while 2.6 is in the 95th percentile, it is still technically normal. It did increase my odds slightly; based on my age my risk of a T21 baby is 1/800 and with the larger NT it is 1/500 for that twin. How old are you? I am not sure how they figured 1/200 unless they are also considering age. Also, how far along were you at the ultrasound? The NT peaks at 12.5 weeks which is exactly where I was when I had it done. 

    All that to say, I understand your concern and I have been worried too. My MFM is conservative and she strongly urged me to wait until the anatomy ultrasound before making a decision about an amnio.  She said if the ultrasound is clear, that will better my odds by 50% to 1/1000. If there are markers at the ultrasound we will likely do an amnio.

    I'm only 25 and at the us I was 11 weeks. I thought my ob was being a little over catious. I really don't see anything wrong with the numbers. DH is worried about it and I keep trying to tell him that it's not a big deal. That's why I posted this.

  • so sorry this must be super stressful.

    i would say just go to meet with the MFM, it is nice to establish that relationship early, but remember most MFM are conservative, there job is to be ready for problems.  we had a terrible first encounter with ours, but we now feel really empowered by the knowledge we have and we feel like we are part of the plan. 

    my MFM has also recommended that i see a therapist during this process, they found one for me who works with MoMs and she will work with them (i have a history of depression they want to watch). but it is nice to have a team. I like to be informed, I don't think your numbers are much to worry about, but if you are going to worry, figure out what is best for and get the support that works for you.

    Well, I am 30 so I think the ratio of 1/200 is not accurate for you. I do agree that seeing the MFM is a good idea, if for no other reason than to ease your fears.

  • My babies results were 1.3 and 2.3 and they told me the results were great. Your results are fine too. I'm 32 and I am not worried.
