dumb teething question

I want to document LO's teeth on a teething chart in her baby book. Do you document the first sight of the tooth cutting or when it fully emerges?

TTC #1 April 2009 Dx with PCOS September 2009 TI w Clomid x3 = BFNs IUI w Clomid = BFN TI w Clomid = BFP! August 2010 We are blessed. TTC #2 TTC on our own August & September 2012 = BFN x 2 (so I thought) Appt w RE Oct 2012 and got a BFP that day! Thank you, Lord. Nov 23 7 week u/s - so far so good.     I marked down when I first felt it poke through. That was his first tooth. My mom just called me and asked me "why didn't you tell me DS got another tooth?" Ummm I had no clue the next one popped thru..so now I have to mark that one down today. So basically I'm all over the place!
