I second the menstrual cup. I have tried diva and lunette, and they are both the shiz. More comfortable than a tampon, more reliable (I never leaked), cheaper in the long run, & you don't have to worry about remembering to put more tampons in your purse. Plus they stay in place.
There is a slight learning curve though, but it didn't take me long to learn how to insert & take out. I recommend youtube to learn about different folding techniques,
I have had this issue too. Mine was a combination of using slender tampons and not getting them far enough in. Within a few hours I'd go to the bathroom and it would be threatening to fall out.
I had a c-section though so I choose to believe it's not loose vag syndrome. Ours just squeeze them out. Take that you loose vag, tampon gobbling hussies.
Well it stays in but it migrates to a mighty uncomfortable position within an hour. And I too had a c/s, no labour or progress at all so I have a hard time believing it's my vag. I saw a post on TTGP (I Think) a while back about o.b. tampons being awesome.

Mine doesn't fall out completely but it doesn't stay up where it's supposed to! It is so annoying and uncomfortable. Stupid loose vag, i am shocked that the c-section moms also have this problem. I had assumed that this problem was a result of two hours of pushing my big-headed child out. Hmmm... Off to do some kegals!

Go with a bigger tampon.
Maybe try using a Diva cup? I switched to one last AF and was really happy with it after the first day. (It took some getting used to.)
I haven't had this problem, but I can imagine it kinda sucks.
