Synagis discount program?

We have very good insurance, however, we will still be responsible for 20% of the cost of the Synagis shot which will be around $400/month.  Anyone know of any discount programs through the manufacturer or other?

My doctor's office had me fill out some form for assistance because we have a high deductible plan.  I'm not sure what it's called, but the doc has the forms.  So, it may be worth asking about.
