There are directions here. The blanket I have is pretty small and thin, so I might make a souped up version later.
I based my poncho off of the one on that website, but I did a few extra steps. I sewed an extra time around the face of the hood to make it lay flatter. And I sewed around the entire poncho then turned it right-side out to make the edges look more finished (then whip stitched the opening where I turned it out).
OP, yours looks great though! Maybe I'll attempt another one after the holidays.
I don't know what your sewing skills are like, but I promise that this was super easy to make. It took me ~1.5 hrs start to finish. And the fleece was on sale at Jo Ann's, so I got all of the supplies (2x1 yd fleece + thread) for ~$10.
