Questions for in-home daycare?

Hi all!! We are exploring a possible in-home daycare... Or actually, a girl who keeps 2 kids at her house. I'm clueless as to what questions I should ask her to help us decide. Any help would be appreciated!!
BFP#1 1/31/11 ~ CK came on her due date, 10/10/11!
BFP#2 11/20/12, EDD 7/30/13 ~ heard heartbeat at 6w2d ~ mmc discovered at 8w
1st medicated cycle ~ 6/11/13 ~ Clomid ~ BFN
2nd medicated cycle ~ 7/12/13 ~ Clomid and trigger shot ~ BFN
SHG on 8/13/13 ~ uterus looked good!
Diagnosed with DOR on 8/16/13 ~ AMH 0.27 ~ repeat AMH 0.19
3rd medicated cycle ~ 8/9/13 ~ Femara and trigger shot ~ BFN
4th medicated cycle ~ 9/4/13 ~ Bravelle, trigger shot, IUI ~ BFP! ~ EDD: 6/11/14 ~ heartbeat of 118 at 6w3d ~ mmc discovered at 9w1d

Is she licensed?
Is she CPR/First Aid certified?
Can she provide recommendations?
Does she plan on taking on any more kids/infants in addition to yours?
Will there ever be visitors during the day?
Where will your baby nap? Do you need to provide a pack n play for naptime?
Will she stick to your instructions for LO's sleeping/eating schedules?
Are there any pets in the house?
Does she take the kids on outings? If so, will they be cleared with you first?
What is her policy for sick kids?
Does she expect any paid vacation time? How much notice will she give if she is going to be on vacation?
Does she provide tax information at the end of the year?
Do you have to pay on days your child isn't there (planned days like vacation or a holiday)?
