We did. DD's birthmom gave her a name just for hospital and SS records, but was aware we would likely change it. We discussed our name with her and she (fortunately) loved it.
K's adoptive parents changed her name. I gave her one name in the hospital, even filled out the SS forms. When the adoption became final they legally changed her name and got a new SS. I knew they were going to call K a different name and I was ok with it. They are her parents and get to name & call her anything they wish, it is their right. She is always my K and will always be their J.
We changed her first name and kept her middle name. Her birthmom had asked us what name(s) we liked. She liked the one that we like the most (Zoe). She suggested the middle name because it's her/her mom's middle name. We liked it and decided to use it.

Amom, Adad and I agreed on a name while I was still pregnant (both first and middle name) and that's what I used in the hospital and she is still Mckenzie to this day :-) Her middle name was the same as my middle name but I changed my middle name to my maiden name when I got married.

My AP didn't find out what my BM named me until I was almost a year old and even then it was by accident that they found out. My AM really likes the name my BM picked out and said she would have kept it if she had know.
My DH adopted my DS when he was 4. We obviously kept his first name the same but we added DH's middle name as an extra middle name for DS and gave him DH's last name.

Literally every person in the agency asked if we were changing it, even before the plan was officially adoption (they asked like, "If you were to get to adopt, would you change the name?"). Except one woman who said, "That's my granddaughger's name. I hate it! You'll change it, right?"she'll. Lol.

Birthmom put a name on the birth certificate and we completely changed it. We never met birth mother so we never discussed it.
