I think Stephen is the "real" spelling, while Steven is the more modern spelling, so I picked Stephen. I would never think to pronounce it "Stefan" unless it was actually spelled "Stefan" (which our HS German exchange student did).
Although Steven has been around since the middle ages, it is still the variant. And although it is also a perfectly fine spelling, I much prefer the earlier spelling- Stephen.
And I don't think Stephanie is a problem. It is the feminine version of the name. It is supposed to be similar. Just like Theodore ->Theodora, Dennis-> Denise. No one would say, "I'm going to name my son Ramown, because Ramone is just too close to Ramona."
My dad is Stephen and he always hated the spelling. People would joke and say it like "steffen" or always spell it like Steven. I prefer the look of Stephen, but then the NN Steve annoys me if it comes from Stephen, since it looks so different. JMO.
