Trouble TTC

XP:2ndIF- RE says NO INJECTS W/O IVF??

I responded OK to Clomid... multiple smallish follicles [15-18mm] and low P4 [4-10] each cycle.  
We did 5 cycles of Clomid before I asked about injectables.  The NP said the only way was if I did IVF because I could have up to 40 eggs.  She annoys me so I just went ahead with the first cycle of femara and then talked to the RE about it when the next cycle started.  He again said, basically, that since I'm "so young" [25] that my egg production is too good... I'd hyper stimulate on the drugs.  He's never pushed IVF [or IUI for that matter] on us.  He's always SO confident that we'll just "hit a pregnancy eventually".  But I think that whole stance is a little outrageous.  Am I wrong?  
I'm responding better the the Femara.  I have less follies [1-3] but they are larger [18-27mm] each cycle.  And my P4 levels are better [around 15].  But, I've been there for a year and the only Dx we have is stage I Endo that he said cleared out and shouldn't even be affecting ttc. I'm ready to get more aggressive.
What would you do?  Do you have any information about injectables that I can take to him to help my case?  Should I just stick to his plan of "eventually it will happen this way"?  Should I have DH's SA ran again?  Try a post-coital test?  Have another Lap??
Finding a new RE is out of the picture as he's the only one Tricare covers out here.  So, until we move in 2 years, he's what we've got.

  • Someone recommended a phone consult with another RE for a 2nd opinion.  I'm definitely going to look into that this week.  Just looking for more experiences over here, if there are any:)

  • Many people have "stronger" follies, better lining, and better p4 levels with injects. And if properly monitored and adjusted, there is no reason to believe that you'd overstimulate. I only had 1 follies come IUI for my IUI+injects cycle even with PCOS and a large AFC. 

    And if IVF is in your future I'm a big supporter of doing at least once injects+IUI cycle to see how you'll respond to the injects as a "practice before the big show" kind of mentality.

  • That seems odd to me. I'm starting my first cycle of injects on Wednesday. I have a great AMH and respond well to clomid, so my RE is starting me on 75iu of follistim. She said this is half the dose she often prescribes, but doesn't want me to overstimulate. I go on Saturday for an u/s and she will increase my dose if need be. I don't see why your RE couldn't try a similar protocol? I'm fairly young also (28) and I don't see why your RE would deny you this just because of your age.

  • I was ready for IVF and to skip an injects cycle, but I was basically the same way... great responder to clomid.  I also have a very high AMH (7.4)... my RE was worried I'd get through all the shots and have to cancel my IUI if we did injects and highly recommended that we switch to IVF...  We also have MFI, though...

    FET#4 12/11/2015 - BFN - 4 blasts remaining
    FET#5 2/18/16 - BFP!!!  Beta1-3, 126, 250, 745!!

  • Of course that is a possiblity that you could over stim and with IVF they feel safer because they are removing the eggs. My first injects cycle was going slowly on a low dose. Then we upped it and i took off like a rocket. It turns out i had to coast for about a week to get my estrogen levels down. When they were safely down we did another u/s to confirm the follies were still there, we did a lupron trigger and just did TI. Turns out because of the coasting that the lupron did not make me ovulate. It was then we decided to move forward with IVF because there was no "safe" dosage for me. Best of luck to you and im sorry your RE wont give it a try. There is no harm (unless you are OOP) to try a cycle, if you over respond just coast it out and don't trigger and abstain. Maybe you can suggest that? GL!
