Size of follicles @ trigger for IVF?

Yesterday my dr told me he was 90% sure I'd be triggering tonight because I was so close yesterday. He cut down my dosages for my shots last night and this morning. At this morning's ultrasound though, literally nothing changed from yesterday. They are waiting to see if my E2 continued to rise and if it did then I will probably stim for one more day. If it hasn't risen then they'll assume my follies are done growing and have me trigger. Either way my biggest one is a 20 with a handful from 15-19 and a bunch of smaller ones. How big did your dr want your follicles to be before triggering? And just out of curiosity what was your E2 when you triggered? Mine yesterday was over 3000.

It took 44 cycles, just over 3 years, 6 failed IUI's in MI, and 1 round of IVF at CCRM to get our BFP!

  • I'm a Cornell patient, and their "thing" is trigger smaller. They like to trigger @ 18 for the biggies. They suspect the others will catch up and be mature @ ER. It worked for me...12 of 13 of mine were mature.

  • I am pretty sure I triggered too late for my 1st IVF - my lead follicles were already 23-25mm and my E2 had started to plateau. There is pretty strong evidence that if you wait more than 24 hours past when the lead follicle reaches 18-20mm to trigger, you will indeed probably retrieve a larger proportion of mature eggs but there is decreased endometrial receptivity for implantation. The thought is that this is a result of letting the lead follies go past their prime because they begin secreting progesterone and then the lining is out of phase with everything else going on. In short, I think it is definitely good to trigger a little on the early side if anything. Good luck!

  • I triggered when most of my follies were around 16-18 and my biggest was 20. Your follies sound perfect! Good luck!
    Diagnosis: MFI and Egg Quality Issues IVF#1-October-BFN ER-16 eggs, 8 mature, 7 fertilized, 2transferred and no frozen IVF#2- March-Beta 3/19 ER- 12 eggs, 8 mature, 7 fertilized, 2 transferred and 1 frozen-BFN. IVF#3 July ER-19 eggs, 13 mature, 10 fertilized...waiting til Day 5 to PGD. PGD results-4 out of 5 embryos are normal and will be banked.   • My E2 was almost 10,000 but that is def. on the high side!

    I don't remember how big all the follies were but my RE waits until at least 4 hit 18mm and then he triggers.

  • My RE triggers when the majority are around 18-19mm, for my first ER my E2 was around 5,500 and wasn't so smooth, didn't get as many mature eggs as expected, and for my second it was over 8,000. My RE suspects I have roughly 500 units of E2 per mature follicle (higher than average) and based on my E2 levels and egg retrieved he is right.
