Hospital Registration

At my last appt (28 weeks) we were given "The Birth Experience" pamphlet to read over and complete a Pre Admission Registration form in the back and return by 30 weeks. Sending it out in the mail as we speak! I think it just hit me we are getting down to the wire soon lol! There was also a "Birth Certificate Registration" and "Labor Preference" form we had to complete and have been instructed to bring to the hospital once the time has come!

Anyone else been requested by there OB/doc to complete a Hospital Pre-Admission Registration?

  • Yes they gave it to me when I was about 16 weeks. I still havent heard anything back from the Hosptial so I guess I should give them a call.

  • Yes my doc has me do it by 28th week just in case of emergency it makes checking you in ALOT faster. Its nice to have that out of the way.

  • Yep.  We did ours on Monday when we took our hospital tour.  I can't believe we are down to the last weeks!!  I am feeling very overwelmed and unprepared!!! 
  • Thanks for the reminder, we got ours at our last appointment, and I still need to fill it out. Maybe thats what I'll work on today so I can take it to my appointment next week.

    Don't feel bad chickie... I havent done a hospital tour either. We have our birthing class next week and supposedly we get the tour then so we will see!

  • I did mine at the hospital a few weeks ago when I was sent there for monitoring.  I was just telling my husband that I have yet to have a pregnancy where everything wasn't completed prior while at the hospital--with each kid I have had some sort of monitoring, fluid checks, etc at the hospital beforehand.

    It's crazy how fast it is all coming :)

  • Haha! Nope, haven't pre-registered yet... and I work here! I'll be doing it soon, though. It is very important and convenient.

    One thing that I will suggest is that you make sure you are all set with Admitting as well as with L&D, sometimes the two departments don't jive very well and they will have you do registration/paperwork stuff for L&D, but leave out/forget to tell you to sign the papers for Admission. At least that happens a lot in my facility. Just make sure you are getting the COA (conditions of admission) signed as well as verifying your demographics and insurance information.

    I can't tell you how many ladies we have come in who swear they pre-registered, but we don't have their pre-signed papers, updated information or a pre-registered account in our system and we unfortunately have to go bug them while in labor. I freaking hate that, I feel so bad that I have to be in their face and many times can't ge to them until they've delivered. So moral of the story, make sure you are completely registered with all departments!

  • Yes they gave it to me in a packet at my first appt and told me to complete it around 28 weeks, so this Friday I have an ultrasound scheduled and my 1 hour GTT and plan to drop it off at admitting then.
  • I believe we have to pre register, but not sure when we'll do that.

    We haven't taken our hospital tour yet either, probably will do that towards the end of October.

  • Seeing as this is my 3rd baby, the hospital already had my info, and just called to verify it all. They will most likely be calling back again to confirm my c-section date that my OB set up.

  • Crap. We got the pre-registration sheet at our very first OB appointment at 7 weeks. I haven't thought about it since. Good thing we have our hospital tour this Saturday. Maybe I can just fill it out then.

  • At my 24 week appt I was given the birth certificate paperwork to mail in but that was it.  I think they let us pre-register at our childbirth class but I'm not sure.  I should probably ask my OB next week.

  • I just got my packet yesterday!! at my 28 week GTT appointment! Big Smile Time is flying by!!! It's crazy!! I haven't even scheduled our Hospital Tour! I need to get to it!!

  • Ya, we need to pre-register.  You can do it online, so I don't feel stressed about getting it done.  Plus, we are hopefully moving soon, so I'm waiting to fill it out until we know what's going on.  My goal is to do it in the next few weeks.  But my doc didn't say anything about filling out birth certificate stuff??

  • I read your post this morning and it reminded me that I am supposed to pre-register. So I spent about an hour trying to pre-register on the hospital's website, but their stupid program keeps getting an error. The error happens at different times during their process, so I'm positive it is on their end, not mine. I'm really frustrated with it. I'm going to give it one more try and then I'm calling them.

  • She told me to do it online (which I thought was awesome that the entire hospital registration was done online). Once it was done and I hit submit, there was this eerie feeling that OMG it's almost here. I am excited, but a little nervous about what it all means.

  • I actually got all that good stuff on my first appointment at seven weeks. I filled mine out about a week ago online. Its crazy to think how little time we really have left till our babies arrive!

  • We were told when we hit 3rd tri to go online and pre-register. I did that a couple of weeks ago.

  • Yep, hospital pre-registration, birth certificate info (as much as possible), authorization for physician's care, and spokesperson designation (I forget what that one's called, but it's the people who you elect to make medical decisions for you, should you be out of it).  All sent in at 28 weeks.

  • My OB didn't say anything yet, but I have already been to the hospital once for the emergency room while pregnant, and twice for ultrasounds. I did fill out the online registration, but I'm not sure if I even needed to. I just found the form online and DH and I filled it out together.

    You're not the only one. We don't have our baby class and tour until Nov 19th...I'll be 35 1/2 weeks but it's the only time that worked! I think I'll ask at my next appt in 2 weeks about calling and registering before then just in case...

  • Yes, I got the paperwork at my last doctor's appointment and have to bring my registration paperwork and my birth plan with me to my appointment next week, so they can already have it in my file, ready to go for when we walk in to have our baby girl!

    I was so excited to fill everything out because it just reminded us how close we really are! 9 weeks left! :)

  • Yeah, I think it's pretty common. We were told to finish ours at least 90 days before my due date. We were able to do most of it online and then I had to fax in some forms separately.

  • Yup, I did it at my 24 week appointment.  Super easy.  At my hospital I think they have you pay your co-pay too if you have one but I deliver for free.
