Questions for specialist

I'm going in for my initial consult with a fertility specialist this Friday with DH. I have no idea what to expect, but am thinking it's going to be a lot of info and talking. I know I can get overwhelmed with info, so I'm trying to write down questions I have now so I don't forget to ask the Friday when my head is spinning. I always walk out of appointments feeling like I forgot to bring something up...so What did you ask? What's important to know? Any advice or suggestions you can offer would be much appreciated!:)

What further tests would you need to run before we started treatment and how much are they?

What would be your next step for me? (what kind of drugs, monitoring, and cost)

In general what kind of success rates do you have for a cycle like the one you described above?

Who would I see for the majority of my appointments? (one dr or rotaiting drs?

If you don't have full fertility coverage, ask if some of the routine tests could be done through your primary dr, such as labs, HSG (ob/gyn), etc so that you don't have to pay the fertility price, and instead have it covered by your insurance.  Also, some of the prescriptions, like BC Pills for an IVF cycle can be prescribed by your ob/gyn instead of fertility to save money.
