I have dealt with it in a classroom setting.  One of my students during my second year of teaching kindergarten had unilateral microtia.  He did not have hearing on his left side because of it and so I had to give him preferential seating (next to me with right side towards me) during large group and small group activities.  His mom let him wear his hair long to cover it up.  When he was feeling particularly trusting he would move his hair to the side and let you see his "little ear".  The kids in my class that year were great and could not have cared less that he was different.  He did very well in school and did not have any speech issues due to his hearing loss.  I think his mom was going to wait and let him decide whether or not he wanted it cosmetically corrected or not, if I remember correctly.
