I've been thinking the same thing the last couple weeks. He's in size 4 but has been forever and ever. Size 4 is 20-something up to 32 lbs I think. But size 5 is 27 lbs and up in Pampers. I guess they assume after that they'll be potty trained...HA! He wears the Pampers overnights one size up (so size 5) and they still seem big for daily use.
My DS has always been average weight (at 18 months he was 28 lbs) and in the 75th percentile for height since around 1-year-old constantly if that helps.

DD was 90% for ht and wt at her 18 mo appt. She has been wearing sz 4 FOREVER. Just the last few weeks she started leaking out at night so we went up to a sz 5 for at night. We almost always buy Up&Up.
Wow, my 1 year old DS has been in size 5 Huggies for about 2 months now! He is 98th percentile plus for height and weight (33 1/4 inches and 31.5 pounds!)
Mine is only 18m, but I'm going to answer anyways. He's currently in pampers size 3. I generally buy Baby Dry, but I had some good coupons and bought Cruisers size 3 recently. It surprised me how much bigger the Cruisers are (I'm not sure what the pounds are, but they came up higher on him). So there obviously is a variance between brands and even within brands.
Re: What size diapers is your 21 month old wearing?

Size 4, Target brand
He's in underwear during the day, but these work great for naps/bedtime.
We've been in a size 4 since he was 12m old. We moved to a 5 in the overnights.
Well, I lied actually, if I use Up & Up we are in a 4...if I use Huggies Little Movers, we need a 5 & Pampers Cruisers we can use either.
