Infertility

Different Protocol questions

Obviously, I know everyones situation is so different.  I don't have any friends that have done IVF, so nothing to compare to.  I am wondering if any experienced people out there would be willing to share what protocol they have done?  For example, I was fully expecting to be put on BCP prior to stims, however, I am taking Estrace.

I guess I am jsut curious as to what, if any, is the most "common" protocol out there.  Right now, my list of drugs is kinda Latin to me!!

I am always so curious to the medical stuff, google gets me in trouble.  It just amazes me at how many different aspects there are to this process, yet they still try to lump everyone together to create sucess rates? LOL

Re: Different Protocol questions

  • What medicines are they having you stim with?  I did take BCP for 21 days.  Then after AF, I've been on Bravelle and Menopur.  I started Ganirelex on Saturday.  My ER should be later this week.  There are so many medicines and they are very overwhelming.  Good luck!!

    https://www.ivf-indiana.com/education/ivf-patient-info.html

The first one is a general overview of popular IVF protocols and the second one gets into a bit more detail - including meds used.

I have done an antagonist and an agonist protocol and used estrace, not estrogen patches.  REs will tailor to the individuals situation and will use what they are most familiar with.

     Good luck.

  • I was on an estrogen patch and ganirelex before AF, then started stims ( 4 vials menopur and 300 follistim a day. Mine is a version of estrogen priming protocol. It's for poor responders.

As you said, there are tons of protocols and it is dependent on your situation.

I was on BCP for 24 days and started Lupron 5 units twice a day on day 21 of the BCP. 14 days after I started Lupron, I added my stimulation meds, Follistim 75 units and 1 vial of Menopur and cut the Lupron in half to just 5 units at night.

I continued with this (5 units Lupron, 75 units Follistim, and 1 vial of Menopur) until stimulation Day 10 and 11, where my Follistim was decreased to 50 units and only used 1/2 vial of Menopur). On Day 12 of Stimulation I only did 50 units of Follistim and Lupron--my HCG trigger was the next day.

They said that my protocol was the "Meldrum Protocol".  It is all so very specific to your particular problems, etc.  I have PCOS and they monitored me very closely to make sure that I didn't get OHSS or have my estrogen increase too quickly.

This one is close to mine.  I feel silly saying this, I keep telling myself just one day at a time, but reading some of y'alls posts makes me feel so silly and under prepared!  I am so thankful for those willing to share with a first time girl.  It's a good thing that there are Drs who know what they are doing.  Better them than me.....

One day at a time is the only way to get through it for me.  Just keep reading and you just pick stuff up.  Some doctors are better than others so you need to be your own advocate and learn as you go. It's hard but you can do it!

  • I have done two different protocols for IVF.  I am dx DOR so probably the opposite protocols than you would use with an Endo dx.

    1) BCP for 12 days, Lupron Microdose Flare, 300 gonal F and 300 Menopur (this cycle was not successful and was canceled)

    2) Estrogen Priming patch, Ganerlelix, 300 gonal F and 300 Menopur (this cycle produced 5 follicles and eight eggs)

Best of luck to you  :)

Actually, I am shooting to be a textbook study, in addition to the Endo, I have super low AMH/ high FSH, very low Antral Follicle count and (newly diagnosed!!)Cervical Stenosis.  And my DH has three different MFI. (we pretty much win the prize for being the craziest medical stories!!) That's what got me to thinking about how amazing it is all the combinations of treatments for so many women!
