I just got back from our consult with Dr. Esposito from Shady Grove. It was very interesting and I think she had some good plans. First we went over all of the paper work from my old clinic, and it turns out they sent a very abbreviated version of my file. Stuff like my HSG report, DH's SA reports and our infectious disease testing results were missing. So I will have to call and hound them to send me those before our next consult with SIRM. We went over all the specifics of my cycles and she said she was impressed with how much I knew that wasn't in the file.
She said that since I am allergic to Lupron we really have no choice but to do the antagonist protocol again. She thinks that I have DOR, which is more than I ever got out of my old RE who never gave me an actual diagnosis. But based off my age, AFC, elevated E2 levels on day 3, and the poor response and fert rates from the IVFs she would bet on me being DOR.
She wants me to re-do all of the Day 3 testing and the HSG since I haven't had one in about two years. She also suggested genetic testing just to be safe.
She wants to put me on their "implantation protocol" Basically I would be on steroids and lovenex starting half way through stimming till 6 weeks of pregnancy. She also wants to play with the ratio of stimming drugs: more menopur and a little less Gonal F. She would do 20,000 units of HCG and do the trigger at 37 hours instead of 36 since my eggs seem to be mostly immature. I asked about DHEA and she said they don't prescribe it but if I wanted to take it she wouldn't stop me.
She also wants to do ICSI on all of my eggs. I told her about what happened with my current RE and the embryologist deciding to not ICSI my eggs last time with out discussing it with us. She said that would not happen here. She assured me that their embryologists are some of the best in the country and they are experts at ICSI.
She also wants me to lose about 30 more pounds, which was already my plan.
I asked her about possibly using my sister as a gestational carrier, her opinion is that the problem really isn't my uterus, that it is my eggs and that if we can just manage to get more good quality embryos from my eggs that we will have a good chance of it working with me.
Overall I felt really good about the appointment and I really liked what Shady Grove has to offer. We have a consult with SIRM in September and hopefully by the end of September we will make a decision about who we are going with and cycle in December or January for the last time.

Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)
IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN
IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.
IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132. Lil is here!
TTC#2: Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.
IVF #4: BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN

I am glad that you guys got some more clarity from Dr. E. I've heard really good things about her. While you are waiting for your SIRM appointment, are you going to get the lab work done that she suggested?

It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.
