Help with Siggy

Good Afternoon eevryone! i'm trying to figure out how to separate the information about me in my siggy (TTC Since, Diagnosed, etc...) On my signature it jumbles all of the lines to one long sentence, but I want each section to have it's own line. Can anyone help out?
