TTC After a Loss 6 Months+

update on my IVF cycle - not going well

I started stims for my IVF cycle last Friday and when I went in for b/w on Monday my E2 wasn't very high.  So they had me come back on Tuesday and it went up enough that they felt things were going okay, I seemed to be a day behind where I was with the last IVF (I didn't have any follies over 11 yet and that is when they start counting them). 

I went back in yesterday and my E2 rose some, but only 1 follie over 11 - it was 14mm on my right side.  I had another 2 that were at 10, a 9 and an 8 and on my left a 10 and an 8 or a 9 - I can't remember and a couple of smaller ones. 

When the nurse called she said that my RE said that things were not looking promising.  That I just don't seem to be responding very well.  As a comparison, last cycle I had 4 follies over 11 on day 6 of stims, this time only 1 follie over 11 on day 7 of stims.  Now my E2 started out lower at baseline, so I am not sure if that is a factor. 

The nurse said I had 2 choices, to either cancel right now, or stim for 2 more days and go back in tomorrow morning to see where things are.  DH and I talked and decided to stim for another 2 days.  This was supposed to be my perfect timing cycle - off work for summer and not stressed.  UGH!!!! 

I hope when I go back tomorrow things are definitive.  I don't want to have to be on the fense about things.  If those other 6 follies come up, I think I will feel okay, but if not then I am not sure.  They said we can still do TI if we cancel.  I won't do an IUI because it would go against my insurance.

The only good thing about canceling would be that the monitoring won't go against my insurance and we can try to cycle again.  I only have enough insurance coverage for a partial cycle - we have to pay OOP for some of it.  This will be it for us treatment wise, we can't afford a 100% OOP cycle. 

So what are your thoughts ladies?  WWYD if you were me?  How many follies would it make it worth proceeding?  Last cycle I only got 5 eggs and only 4 fertilized and only 1 made it to transfer.  I was really hoping for better results this time.  Crying

  • Oh love, I'm so sorry. Now let me ask you a couple of questions- your protocol was different this month, right? You didn't take the BCP's last time, right? Could it be that they suppressed your system too much this month? I agree with you giving it two more days to see what happens. I can't give you a number of follies, that is a very personal decision as it only takes one. I say listen to your heart, maybe in two days all of the other follies will have caught up and  the decision will be easy!

    Thanks hun.  Yes, I am doing the antagonist protocol this time, it was between this and the micro-dose and I decided on this with the lupron shortage and all.  My RE thought either would be fine.  Of course, now I am having regrets. 

    Anyway, you are correct I was not on BCP's last time, just lupron (long lupron) and I was on them this time, but only for 2 weeks.  Lupron is supposed to be so much more suppressive, but my baseline E2 last time was 43 (just under the 50 they want to see) and this time the baseline was 22 - so much lower.  The nurse told me that my dr said she didn't think I was "too supressed" based on the numbers, but obviously something isn't right. 

    I know no one can tell me how many follies to proceed with, it's just hard.  I guess I was hoping for a miracle and to end up with like 10 eggs.  Last time I had 7 mature follies at trigger and they ended up getting 5 eggs.  Of course, only 4 fertilized and only 1 turned out okay and that still didn't end up good in the end.

    I talked to my coordinator today and asked her some questions, she said they would proceed with as few as 3 maturing follies.  I asked if they were so slow starting did that mean they would be poor quality and she said no.  I am not sure if I would want to proceed with just 3 though, most likely not.  I feel like if there are 6 that are maturing, that would be a good number.  4 or 5 is a gray area to me.  I guess we'll see what tomorrow brings.  Thanks again.   :-)

  • I can't answer your question, but I wanted to give you (((HUGS))).  I hope that those follies grow and you get good results at your ultrasound tomorrow.  FX for you!

    Could you stop the cycle and maybe do a bit of acupuncture and metaformin to work on egg quality before doing another cycle?  I know some people have had success with that and metaformin (5 mg taken nightly before trigger) is suppose to help increase the egg quality.

    I'm not sure if I could do that.  I think the problem would be that I would have to then pay OOP for all of this monitoring too.  Things look a bit better today, but we'll see what they look like on Monday.

    I did an update above and things have improved.  The RE has never mentioned metaformin.  I know that people who have PCOS take that, but I didn't know it did anything as far as egg quality.  Hmmm... interesting.  I guess maybe I should ask about that.  I am doing acupuncture.  I did it for a few months this fall/winter.  Then I stopped after the last IVF, but I've been going back for a few weeks.  I guess I'll see what they say on Monday.  Thanks for your thoughts!!

    Bearing in mind that I have no idea how your insurance is structured, the impression I'd gotten from your PP was that if the ER and ET are cancelled, the monitoring doesn't count against your insurance allowance AND will still be covered by insurance.

     I was figuring then that doing IUI OOP means the insurance never hears that a fertility treatment *was* given, they only hear that one was cancelled - like they would if you just straight up cancelled. 

     Or I might be talking about my arse. I dunno.
