Consent for Med Tx, while on vacay?

Does anyone have a simple one I can have?

 

I used to have one, cleaned up the hard drive & must have wiped it out. :(

All I'm able to find online are the ones for childcare facilities and I don't need all of that legal jargon cause it's just for the IL's.

    I don't have one but my sister always leaves me one (a very short one) that states that I have permission to care for her children while she is away and make any medical decisions in her absence. She then adds each child's name and medical record number.  She signes and adds her cell phone number to it.
