I have heard people say they use a sleeper (usually fleece) over their soakers, but I would think a cotton onesie would cause a risk of wicking and/or compression leaks.
Usually nothing, we put DS in longies if it's cool and when it's warm just the soaker and a t-shirt. We have put knit pants over the soaker before, and it works with no compression leaks, so I imagine you can do whatever you like.
Nothing. Lately it's been super hot here so he's either just in a diaper and soaker with a shirt. He has the hottest room in the house even with AC on so sometimes i won't even put a shirt on him and he's fine.

DD wears her overnight fitted, topped with a wcw soaker, a onsie over that and footed cotton pjs without a problem

As long as it's not too tight, I've put sleepers, pants, etc. over her wool.
