Reoccurring diaper rash

On and off for over a month now, the tip of DS' penis will get red and look like a couple layers of skin have been removed. No other areas of his skin are affected. I put Grandma El's on it and eventually it seems to go away. But will come back the next day or a few days later.

Most of DS' CDs are pocket diapers with microfiber inserts. I've used vinegar in the rinse cycle, RLR treatments with multiple rinses and I even bleached them with lots of lots of rinses afterward. Part of my problem could be that I haven't done all the inserts at once so I can't guarantee all the inserts are ammonia or bacteria-free.

I plan on washing all of his inserts together with RLR and then maybe with some bleach this weekend while he's at my in-laws' house overnight. But does anyone have any other suggestions?

Could it be a sensitivity to the soap?  Also some babies have sensitivities to the microsuede in BG.

I've used the same diapers (BG and SunBabies) and soap (the Eco-Friendly Family CD detergent recipe) for the past 8 months.

Are you tucking him down? Could it just be a contact/rubbing type of thing?
