Diaper rash question. Help!! (long)

We have been battling a bad diaper rash on and off (mostly on) for the past couple weeks.We use cloth diapers with DD, but disposeables every now and then when we have to use the Calmoseptine diaper cream her pedi prescribed. I will say, whichever we use with her doesn't matter, she still gets this diaper rash and it won't go away.

It all started because she switched her pooping schedule and pooped in the middle of the night and we didn't know until the morning. She has done it occasionally over the months and usually wakes up very restless and won't go back to sleep, so we can tell that she needs to be changed. Well, lately she has been doing it more often, and not alerting us.

So, when it started, she had slept in a poopy diaper all night and when I took it off her in the morning, she had this nasty red rash with red bumps all over the place. I was very vigilant about the diaper cream, and we put her in disposeables for a few days to save our cloth diapers. It was clearing up slowly but surely. Then when it was almost healed, she pooped in the middle of the night again, didn't alert us, and the next morning it was back again. We have been repeating this cycle now 3 or 4 times. We get it cleared up, put her back in her cloth, and it happens again.

Now before it comes up, this isn't happening just by putting her back in her cloth. If she just has a wet diaper, her butt is fine. So I don't think it is a detergent or cloth issue. It is only after she poops and has an extended contact with it. Tonight, she pooped while napping, and I went to change her after she woke up. She hadn't even had the poop in contact with her for 2  hours and we are right back to square one. Red rash, red bumps, back in disposeables. I don't know what to do! The pedi has given us a couple different scripts for this and Calmoseptine works beautifully, but I don't want to have to keep treating this and keep having to make her wear disposeables (I have tried liners in our cloth, but she is SO active, the crawling dislodges them and it gets on the diapers).

Does she just have really sensitive skin? Can she be allergic to something in her poop? I am at a loss, and I am so over dealing with this.

  • Have you tried to use A and D even if she does not have a rash to help keep any poop from direct contact with her skin.  That seems to work for us.  I just put a little on after every diaper change per pedi.  We were having some rash problems for awhile but have not had one in months.

  • Even 30 minutes of poop contact usually results in a bad rash for us so I am not surprised by 2 hours resulting in a bad one, poor baby and poor mama - tell that little girl to quit poopin' at night!   Poop is just very acidic and prolonged contact with it just EATS their bum skin - so it's not her having sensitive skin - it's the prolonged contact with the poop. 

    I would keep up with calmoseptine as needed - and also try FROSTING HER BUTT with Triple Paste at night before bed - you should be able to press your finger into it and make a finger mark - that thick.  I would do this to help continue to heal but also as a barrier as much as possible.  Aquaphor could also work but I'd try triple paste if you can.  See if that helps - she'll still get a rash with prolonged poop contact but not as badly.

    Beyond that - until she gets on a different poop schedule, I don't know what you can do.  Jack was never a night-time pooper - only happened once or twice (well, after newbornhood was over) so we've been lucky.  Now he's every morning around 10am BUT sometimes earlier before we're even out of bed so those mornings have caused redness and irritation from time to time.  

    During the day - keep her out of diaper as much as you can - the more naked butt time the better - at least 30 minutes once a day or more if possible.

  • Both of my children will get a rash at the drop of a hat if they sit in a dirty diaper for any amount of time. I don't think there's anything wrong with her other than sitting in the diaper too long. (obviously you can't help this.) I think probably what I would do is use disposables for nap time and bed time, and coat her bottom with a thick, not lotion-ey, diaper cream before putting her down. A&D is too lotioney. I'd personally go with burts bees or something that is almost all zinc. Something very pastey is the key to it not coming off. BB's stays on forever.  :)

  • I might try disposables over night with some type of cream to help form a barrier.  That might save her from a rash even if she doesn't alert you to the poop.  

    FWIW, C had a bad reaction to his poop after he ate citrus fruit for a stretch.  It eventually got better, but poop after oranges or strawberries would cause a rash if it wasn't changed IMMEDIATELY. 

  • (I just remembered this) This may sound weird but I heard that you can put some breast milk on a diaper rash and supposedly helps.  Never tired it but I just remember someone telling me that.  I wonder if anyone has tired it?  It might be worth a short if you are still nursing.

  • We CD during the day but use disposables at night, mostly because then we can slather on the diaper cream. Luke gets a horrible, bleeding diaper rash if we don't. It made me so sad to see him hurting like that.

    I also heard from a friend that you can get pure zinc oxide from the pharmacist, but mine (target) said that's not true.

    Sorry you are both dealing with this. No fun at all.

    Yes, still nursing. It is recommended to put breast milk on sore nipples when first starting to nurse, so I would think it would be just as great for her little bum.

    Thanks for all the replies ladies. She isn't pooping every night, just enough to keep perpetuating this damn rash. I'll look into the other pastes.

    This is what I would do.  Clear up the existing rash while in disposables, and then if a liner won't stay in place overnight, put her in disposables at night so you can use a thick layer of triple paste.  (However, usually if I use triple paste, the liner sticks to her butt really well - we just use cheap fleece from walmart cut into 5x12 strips.)  You'll go through way fewer disposables if you just use one at night and prevent/minimize the rash with triple paste than this sad cycle of rashes.  :(  I'm so sorry!

  • Marlo gets horrendous diaper rashes on and off, it's defiitely related to teething but no amount of preventative measures can stop it. My mom showed me how to make a paste of Vaseline and corn starch and that works well, so does desitin. Boudreaux's also makes this spray that is basically just oil. I slather marlo's tush in whatever cream I am using and then use the spray, it helps protect her skin from the moisture. I feel so bad for her sometimes, it's so obviously painful as she usually does not cry during diaper changes, but when her butt is inflamed she shrieks when I wipe her off.
