What needs to be done.....

We will be out of town for a few days without the boys.  What kind of info/documentation do I need to leave to allow family members to take them to the Dr/hospital if needed and have them treated?  Do I need to have this notarized?

I always left them with a Power of Attorney form.  You should be able to download a template online somewhere, google Free P of A form. 

I always got mine notarized.  The admin assistants at work would do it for me or else I'd take it to the bank.

This and I usually leave their insurance card too. I also give my mom her SSN just in case she needs it to fill out forms

Yes.  I always put both my parents and my sister.  And yes on leaving the insurance card, I always left that too.
