When we ask DS a question i.e. would you like a cookie? he says No when he means yes. I try to repeat Yes to him but not sure he is getting it. Any suggestions to teach him?
DD went through a stage like this- and sometimes still confuses them. I would just give her the appropriate action to the response she's giving. Aka,
Me: "Do you want a cookie?"
DD: "No"
Me: "Okay.", putting cookie back.
DD subsequently whines so I ask her again, until she answers yes and then give her the cookie. If she never says, "yes" eventually I'll give her the answer she's looking for and she'll echo it back to me, and then get her cookie.
I personally think it just takes time. Almost every toddler I know does this. For one, DS can say no, but I've never heard him say yes. So I think he just thinks it is the answer for everything. I think what you are doing is just fine and he will eventually get it.
Maybe before you ask him a question ask to yourself first. Ie: Does mommy want a cookie? Yes. And give yourself a cookie. Then ask him.
LO has her yes and no's down but for some reason no and milk both come out as no. So sometimes I am asking her if she wants something and she says no but she is actually trying to tell me she wants milk. She gets very fruturated with me when I don't get it and melts down.

My cousin is 3 yrs old and everything is no to him still.
Our DD is the same way at 15 mo. I think it's just easier to say "no" than "yes"!
Just keep modeling as some others have suggested, and it should be fine
