What forms do I need to fill out? Delivery at Inova Fair Oaks?

I will be giving birth at Invoa Fair Oaks.  I know I need to fill out a registration form -- does any one know where I can find this and any other documents I need to complete prior to?

I'm delivering at Inova Alexandria and I think all the Inova hospitals have the same OB pre-admission form which you can access here -https://connect.inova.com/j/inovanet.srt/www/OB/OB_PreAdmin.jsp

I filled mine out and got something in the mail a short time later confirming my admission was complete.

Ask your OB--they usually have the forms in their office.  And just remember to send them in EARLY so they have time to process them before you get there.
