Do you think the skull on the jolly roger is done as part of the knit or is it done after the fact? Either way how do you do it?

I would guess it is duplicate stitching. Here's a link to a tutorial, if you google you can find more examples of how to do it:

Hard to tell. If it's worked into the pattern you can create one on graph paper and just change yarn colors as needed.
