looking for Enfamil Rebate coupon

I know a whole lot of swapping doesn't go on for this board.

But this is the board with a lot of "oldies" on it  that I recognize from years ago.  I am looking for the Enfamil Rebate form that was in the Enfamil breastfeeding gift.  Breast feeding went great with DD#2.  I threw the form out when I had it, because I didn't think I needed it.  She is now 7 months old and loosing  interest in breast feeding, so I am having to supplement.  Anyone have one they are not going to use?
