DOOL watchers...catch me up?

Can someone catch up me on these story lines

1. Rafe/Sami/Fake Rafe - why is EJ holding Rafe hostage? Does Nicole know?

2. So what all went down with Bo/Hope? Last I tuned in they were on the run and Jennifer was poking around the prison....and someone needed a transplant, so I figured they were going to do something to Jennifer. Now today I see Jennifer is in the hospital and Bo/Hope are at the police station.

    Nicole does not know. He doesn't want Rafe with the kids, and he's enjoying punishing Sami.

    Bo was taken to a clearing in the woods to be killed by the Warden/Lee. Hope/Carly went and somehow I guess stopped it/got caught (I missed that day), were taken into custody, Carly alerted Daniel what was going on, and Bo/Hope were able to prove the warden was doing shady stuff at the prison. Yes, they took Jen's heart and Daniel/Melanie saved her.
