we just did it for the first time today, but we mixed with breast milk in a bowl. We just followed the directions on the box and did one tablespoon of cereal with three of milk. We are going to do once a day until we go in for their 6 month appointment in a couple weeks and talk to the pedi about it to see what he thinks is best.

When we tried it as a solid, we mixed w/ formula and I basically just added & stirred til it seemed like the right consistency- watery-ish but could still be fed on a spoon and not really like a drink.
Unless your LOs ahve reflux or have been advised for another reason to put thicken formula, there is no need to put cereal in a bottle. It doesnt have much nutritional value beyond iron fortification and your formula already has that, it is more about practicing & getting used to swallowing something thick.
We mix it with formula in a bowl. we started with I think 1 tbs of cereal and about a oz of formula. Just add enough till its a good consistency so its not too thick but not too watery. We do cereal twice a day. We do cereal and fruit in the am and cereal and veggie in the pm. Just make sure you wait about 4 days before you introduce any new foods to check for allergies.
