Maddie went to size 5, but I try really had to find 5 slim. It's only because 4 is too short. Thye HAVE to have an adjustable waist, and are always huge in her hips if they are regular fit pants. Shirts and dresses are usually fine. And everything costs more.
Michelle wears 5 shirts...5Ts are long enough in the torso. She still wears 4T pants

In my experience, size 4 and 4T are essentially the same (size 5 and 5T are too). You could try XS in the girls dept, which is usually a size 4-5. Some stores will have a 5T and some will have a girls size 5.

depends on the store. I'd compare at a store that has both 4 and 4t. at target, the 4 slims are slightly longer than the 4Ts. carter's 4s are about the same length as the target 4s. not sure about crazy 8 or tcp or gap. dd still wears some 3T gap leggings. she's in 4 slim pants, most tops are 5s now. 4t sleeves are too short. dresses....she has 3t, 4t, that fit.... even a few 2 t strappy sun dresses.
We did 5T is possible, otherwise we went to a 4 in Gymboree or a 5 at Carters. DD wears 6 shirts/dresses now but still wears 5 in pants.
For me (and we have limited stores around her), I had a hard time finding clothes that still looked age appropriate but were in the older section. I had the most lost at Target and Kohl's.
It is my understanding that the "T" stands for Toddler and usually means "room for a diaper". If a 4T and a 4 (pants) are available from the same manufacturer, the 4 will be a bit slimmer in the butt and waist and maybe a little longer.
But, of course you know how much variability there is between manufacturers in sizing. I'll often tell clothing buying grand parents "She's a size 4 in Gymboree and Gap, SIze 5 in Target and Kohls....bla bla bla"
