I had a 10k lifetime max through my company last year (with aetna). Luckily the only things that applied toward my max were the actual IUIs, ERs and ETs so my max coverage covered everything last year. (I work for a large healthcare company based out of San Fran.)
I moved to DH's this year who has BCBS of IL. I'm not sure about monitoring appts, etc, but I do know they'll cover 4 ERs and 4 ETs per lifetime- no lifetime max. He works for a parking company.

I'm late to game so I'm not sure if you are going to get this...but I have great insurance and live in Chicago suburbs and work in Chicago...infertility is treated just as any other illness/disease. Out of pocket max of 2000.00 a calendar. There is a limit of 6 egg retrievals lifetime however. I work at a pretty large social service agency--PM me if you need more info!!

11/4/11: The twins are born at 36w4d!
11/5/11: We said goodbye to our sweet baby girl as she was born with multiple complications and a severe heart defect, Hypoplastic Left Heart Syndrome.
I work at a regional public accounting firm in central IL and my insurance is thru Blue Cross Blue Shield of Illinois (BCBS). IL is a mandated state! I have great IF coverage... diagnostic testing was covered, IUIs and other procedures are covered, and up to 4 IVF cycles are covered, with some weird provision that if one works, you can actually get upto 6 IVF cycles covered. Of course, we still have to pay copays and co-insurance, but that is much cheaper than paying for the whole shebang. My pharmacy insurance is separate, but I also have coverage for IF meds. DH got a new job and we moved to Indy late last year, however, I chose to stay with my current employer in IL a little while longer since Indiana is not a mandated state.... bummer. I'm still hoping to get that illusive BFP before I give up my insurance.

Don't know if you're interested in banking or finance, but we moved when my husband got a job with Chase. They have great IF benefits ($20000 max lifetime on IVF/IUI and $10000 max meds lifetime) and we pay a $50 copay. It's a POS I believe with United Healthcare. And because of contracted rates with the insurance, that $20000 will get us just about up to 5 cycles. The meds will be covered for 2.
When my H was looking for a new job and we knew we needed coverage, he started his job search with the top 50 companies for IF/adoption benefits listed in Conceive magazine. https://www.conceiveonline.com/fifty-best-companies/conceives-50-best-companies-2010/Page-1/ He actually got an interview and job offer with Chase a few months after we had looked at this, and when we looked at it again one day and realized that Chase was on the list, he accepted the job right away

ladies that have ivf coverage count your blessings. i live in a nonmandate state and work for the state so i get zero nada paid. everything is oop even blood draw. that sucks big time.

I have Cigna and they pay for NOTHING. Fortunately, DH has Blue Cross and since he's a teacher there's a state mandate that they cover 100%. We are soooo lucky.

I switched jobs to a lower paying job so that we would have insurance coverage. We also gave up a great job in a non-mandate state because we couldn't be sure they would give us coverage at the new job, and it wasn't something we could ask about. So, we are extremely grateful to have coverage-but for us, it has come with consequences-I loved my old job and I took almost a 25% paycut. DH would have loved the other job-but we knew we couldn't take the chance, and so he chose to take this job instead and moved farther from our families (instead of closer!).
Citi has a $24,000 lifetime max for intertility treatment and $7,500 max on medication. We're lucky in that our IVF is only $7500 per cycle, so we have enough coverage for 3 IVF's. After our 2nd IVF in February we'll be out of pocket for meds, since we will meet the max, but I'm so, so thankful that we've had coverage at all.

We're in IL and have my DH's BCBS. He works for a school district in McHenry country. We consider ourselves very lucky.
My company doesn't meet the mandate either because they are self insured. UGH.
