Did your medical records make you laugh??

Here are a couple of exerpts from my chart:

In general, this is a somewhat overweight, white female who appears in her early 30s. She has no gross abnomalities.

Well, at least I'm not gross.  Yay for that! lol

The patient has a good working knowledge of these procedures based on internet research.

    Baby N conceived after 1 miscarriage and more than 2 years of TTC. Diagnosis was low sperm count. We found success after 3 months of anastrozole to increase DH's testosterone and one IUI.

  • I had 2 REs and multiple nurses comment that I was "persistent."  I'm just trying to solve the problem!

    Yeah, it is pretty sad to see everything you've been through in black and white.  It seems so different when you strip all of the emotions away.

    That's what's so crazy...this was part of the dictation summarizing our first consult!!  All of their records are electronic.  I didn't get to see any editorial comments either.  Mine would read "patient cried uncontrollably following 5th follie check after she was told her ovarian response was 'fine'."

  • I haven't seen any of my "recent" medical records, but we did get copies of everything before we moved.  Most of mine was pediactric stuff, so imagine my surprise when a doctor I saw at age 12 (for migraines) described me as "very well developed"!!  Yes, I was a 34C at the time, but that has no bearing my migraines!!

  • I'm sure if you read mine without meeting me you would think I'm nuts.  I'm sure it says that I burst into tears every year when my OBGYN asks how I'm doing and if we're still pursuing infertility treatment. 

    I did see my delivery notes and the perinatologist said:  "Patient is visibily upset, crying at this time.  Responds to questions appropriately"  Uh, yeah.. You just told me that my baby is still and I'll have to be induced and go through delivery.  No crap I'm crying...     I'm not upset about the comment now, but it is forever burned into my brain.   I don't know if I want to know what's in the rest of my chart!  LOL

    Thanks for your kind words, but honestly at this point I'm over it- she was just stating the facts and at least she acknowledged that I was still able to answer questions!  LOL

    This! Although it's been warranted :)

    Many failed IUI's (medicated and unmedicated) IVF#1-Micro-dose Lupron protocol: BFN IVF#2-Antagonist Protocol-transferred 1 8-cell grade A embryo: BFP! Missed Miscarriage- D&C 4/2011 IVF#3-Antagonist protocol again but no bcp's: August 2011 DOR, endo and cervical stenosis ER 8/16/11 3 er 3 fert w/icsi ET 8/19/11-assisted hatching- transferred (2) 8 cell embryos! 9/1/11=Beta#1=824 9/6/11=Beta#2=7337
