Birth Certificate-Parents have dif Last Names

My fiance and I are getting married in August, but our baby is due in March.  When we fill out the birth certificate we will have different last names, but in August I am taking his last name.  We are giving the baby his last name, but how do I change the birth certificate later so my last name is the same as theirs? Thanks!

  • Not sure....but there will be someone at the hospital that stops in your room to go over Social Security Numbers, birth certificates, etc. They will be able to answer your question for sure.
  • Birth certificates are printed with mother's maiden name, not whatever her married name may be. So, it doesn't matter if you were married or not, it would still have just your maiden name on the certificate. They asked me if I was married or not. (I'm not. We are getting married later this year.) My son was born in downtown Chicago two months ago and I just got his certificate a couple weeks ago. Double checking the paper, there is a line of "mother's current legal name" but that can't be changed as that has whatever my legal name was at his birth, not whatever I may change it to down the road.
