Insurance Rejected IVF Claim

I have coverage for 2 IVFs (lifetime) through my secondary insurance, which I haven't used.  My primary insurance is maxed out, so I submitted the claim to my secondary insurance.  They rejected it based on my not getting prior authorization and they also said they were requesting medical records.

I called and spoke to someone who told me that my policy does not require prior authorization and it should not have been rejected.  I told her the clinic actually called to ask if we needed prior authorization (it says we do in my policy), and they were told no.  I even have a reference number for that conversation.

So, the claims are going to be resubmitted.  We'll see.

  • i hate dealign with insurance sometimes.  It's why I've learned to make note of names and dates I had conversations.  Good thing you have the reference number to back it up.

  • I argue with DH's insurance company at least twice a week. They are my secondary, but are refusing to pay for anything at this point. It took them 3 months to finally pay for DH's semen analysis that we called and had pre-approved. After the claim was rejected I called to question it. Every time I called I got names and reference numbers. Now every time I call I get sent straight to a person, I find it quite comical. I imagine they have a flashing light every time they see our member id come up on the screen.
