PCOS dx questions from a newbie

From lurking, it appears that most everyone has been trying for a year or more, so I hope it?s okay that I?m posting here. 

      Hi ladies, hoping you can help me with a few questions. I was diagnosed last month with PCOS, but the more I read, I wonder how accurate that diagnosis is.  I have irregular, painful cycles, 33 days one month, 48 the next.  They did an internal sonogram and said I had lots of cysts, but didn?t do any blood work.  Don?t they need to do blood work to confirm?? Also, due to what they saw and negative OPK?s, they said I do ovulate, but very rarely.  My doctor offered to put me on Clomid right away, but I declined because of everything I have read about not enough monitoring from an OBGYN.  I want to be more informed before I start, and rule out issues with DH.  I go back in the beginning of April, which will be 1 year of actively trying.  Do you think I should get a SA for my husband now, instead of waiting until then, so I am ready to start Clomid if I need to, or should I be patient and wait until the year mark. If so, is this something I just call up his regular doctor for, or does it have to be through a specialist?  Sorry this is long, just not sure how I should proceed.  Thanks!

 

TTC #1 since 4/10; Diagnosed with PCOS 11/10 by RE; BFP 6/11, heard the heartbeat twice. no heartbeat on 7/13/11, d&c the next day @ 9 weeks 2 days. BFP 1/30/12 from IUI, triplets confirmed at 6 weeks. Everything is great until I had pPROM, delivered the next day at 22 weeks 6 days. 1 was stillborn, 2 lived for a short time.   • With a dx of PCOS you should (depending on your insurance) be able to go right to a RE. I was dx with PCOS in September and we had only been trying for 4 months. Because I was charting I knew I wasnt ovulating. My OB ran some blood tests and sent my right to the RE (who did more tests). They ordered an HSG for me and an SA for DH. Its important to rule out both things for you before you start clomid. If your DH's insurance will pay for the SA you might as well do it now... cant really hurt.

    With all tests done I am on my first round of clomid. Its really important to go to a RE for clomid because the monitoring is very important.

  • I agree with JNKS, with the PCOS dx, you should be able to go right to the RE.  They should do b/w on you to confirm the dx, but it definitely sounds like PCOS.

    You were definitely right to say you wanted more monitoring.  Not sure if you're OOP or need a referral for a specialist, but I would start looking for one now and not wait until April.

  • I think its' your call. We started with obgyn 3 rounds of clomid (failed) then went to RE. After talking with the ladies here and going the route we did I wish i would have started with RE. They will handle everything from there.(SA, HSG, Treatment) I hope you won't but for the rest of us we have had to have many different treatments to even grow a Follie.

  • pcos is often diagnosed when you have 2 or more symptoms of it.  in your case, irregular cycles & cysts.  they could do bloodwork to futher confirm it but you will probably get results that confirm it.

    If you want to rule out DH having/not having issues, get the SA.  In our case, we didn't have DH get an SA.  I was fairly certain only I was the issue & once we got me O'ing on clomid, we didn't have trouble getting pregnant.  that said, every situation is different.  we so happened to be lucky to not need to go down that road.

    I definitely don't think you need to wait a full yr though.  When you have  pcos diagnosis, that nullifies the rule.

  • Thank you all so much for your responses.  We have great insurance, so I think we may just go right to the RE, and skip Clomid from the OBGYN.  You're right, I do need more monitoring and it is pointless to keep wasting time if I know I'm not o'ing regularly.  I look forward to getting to know everyone and letting you know what happens.  Thank you!!!
