 For doctors, office staff, and patients who need assistance with patient benefit verifications, distributor triage, preauthorization assistance, denied claims and appeals.
- MedImmune Assistance Program (1-877-480-8082) 
For eligible patients who do not have health insurance (including Medicaid and Medicare) and are unable to get coverage for Synagis. To qualify for assistance, you must apply. Your health care provider can help you, but the guidelines are simple: - The patient must live and be treated in the United States 
- The patient must not have any health insurance (although you may be in the process of applying for it) 
- The household income of the patient?s family must not exceed 400% of the Federal Poverty Level, as published by the US Department of Health and Human Services annually. (Currently, 400% of the FPL for a household of 3 is $73,240.) 
 
- Patient Access Network Foundation (1-866-316-7263)
 For patients with high co-pays and deductibles, this nonprofit organization may be able to provide support.

No.
She is not willing to write me a letter.... so I am going to ask the neonatologist from the Nicu if she will write me a letter to send in with my appeal....
Why are you having to do this leg work? That is something that your pedi should be doing IMO and if it were me, I'd find a new Pedi.
