I didn't need anything. They just asked my address and my name.

I didn't need anything. Just had to state my address.

It depends where you live. In Wisconsin, we don't need anything, you just have to know your name and address. I think some states have more strict requirements.

I didn't need anything. I just had to tell them my name and sign next to a copy that they had of my signature.
