I guess it depends on your state laws and your company policies. In my state they are required to give you time/breaks to pump but they are not required to pay you for these breaks.

If your company has more than 50 employees they have to give you time and a place to pump. If they have less I think your on your own. But the time does not have to be paid. If you can do it during a break time then it shouldn't be much of an issue.
Depends on your company, everyone knew I was pumping and noone told me I had to make up the time. I still take my hour lunch. I am salary though, not hourly so I don't know if that makes a difference. I know with my hourly employees they would need to make up the time.

I pump 2-3 times per day and drive to daycare to nurse at lunch...we have a "mothers room" with a comfy chair and a little fridge. I do get paid, but I'm also salary.
Also salaried, but because I pump 3 times at work for about 20 minutes per session, I rarely take a lunch break.

I pump while I have my lunch. I pump 3 times at work, 9, 12 and 3, for roughly 20 mins each time.
This is what I did too when I was working. However, my lunch break was usually 40 minutes instead of 30 - and nothing was ever said to me. I think it depends on your company.
It depends on your employer and your state. I teach. I asked before I went on maternity leave what they could do to support me. I have a morning plan time and an afternoon plan time now, I pump each of those times. I get 17 minutes for lunch so not enough time for me to pump another time. It's working for me,just barely.
Technically I get a paid 15 min and two unpaid 30 min breaks - one is my lunch and the other is a law-mandated nursing break I'm allowed. But my boss doesn't make me take it unpaid because I only take another 15, not 30, and we seriously have smokers that take more breaks (and are gone longer!) than I do.
I'm salaried and work for a company with <40 employees. I take 2 pumping breaks, usually 18ish minutes each. I rarely take lunch to make up for it - but I was not told to do so by the company.
