I think my doctor's office charged me to fill out my FMLA forms! It's only $10 but that's not the point. Doesn't it seem like they shouldn't be allowed to charge for filling out federally mandated forms (it was the WH-380-E)? Or at least I think they should have told me when I submitted them! Has this happened to anyone else?

I haven't had them fill anything out yet, but if they charge me, I'll raise hell. I'm with you - if it's federally mandated, they can absorb the 10 minutes it takes to fill it out.
Thanks ladies! Now I'm glad that it was just $10. Still seems like it should just be part of the OB's overhead costs but oh well!
HR gave them to me but it was the OB office that charged to fill them out and sign them.
Huh my doctor just filled them out one day during my appointment right in front of me and then gave them back to me to send back to my HR office. I wonder if he charged me and I just do not know it yet.. Sneaky!
This! And I paid $25... They said I could fill them out myself and just have the Dr. sign them for free, but I figured I would screw something up and not get my FMLA. I thought it was a bit ridiculous!
