Charged for FMLA forms?

I think my doctor's office charged me to fill out my FMLA forms!  It's only $10 but that's not the point.  Doesn't it seem like they shouldn't be allowed to charge for filling out federally mandated forms (it was the WH-380-E)?  Or at least I think they should have told me when I submitted them!  Has this happened to anyone else?

I haven't had them fill anything out yet, but if they charge me, I'll raise hell. I'm with you - if it's federally mandated, they can absorb the 10 minutes it takes to fill it out.

Thanks ladies!  Now I'm glad that it was just $10.  Still seems like it should just be part of the OB's overhead costs but oh well!

Huh my doctor just filled them out one day during my appointment right in front of me and then gave them back to me to send back to my HR office. I wonder if he charged me and I just do not know it yet.. Sneaky!

My employer hasn't asked for them, so I am assuming the letter I wrote and form I filled out for my employer will be enough. They haven't stated otherwise.

This!  And I paid $25... They said I could fill them out myself and just have the Dr. sign them for free, but I figured I would screw something up and not get my FMLA.  I thought it was a bit ridiculous!

Mine beats you all...I have to pay $35!!!!! It is such a rip off!!!!

It's federally mandated for you to have it if you want the time off, it is NOT federally mandated for them to take the time to fill it out.  It sucks, but there you go.
