Will these shrink in a hot wash?  My DD is just barely 20 lbs. and super skinny, so I'm afraid that the 18 month size will fall off her.

I've washed them only once so far and they did not shrink much. Fwiw, I've used the 2T size with my 23lb DS with no problems while waiting for the 18 month size to get here in the mail so the 18 month size may work just fine for you.
