I just saw your post last night about clinics and wanted to make sure you saw my response...
I was at St. Luke's too, but w/ Dr. Stein. I loved him, he had a great bedside manner, but I did 2 Clomid cycles and 4 IUIs there w/ no success so I switched to Cornell. Luckily they do take my insurance (I switched providers just so I could go there), but even if they don't, will your insurance reimburse for OON providers? You've probably looked into this, just checking.
Anyway, I've met Dr. Keltz and he seemed fine to me - if not a little removed - but I've heard that some patients have difficulty w/ his bedside manner. Their success rate there is good. I did hear one story though from a girl I had my ER at Cornell w/ who had a bad experience there - she didn't go into specifics, but it had to do w/ how she was stimmed for her IVF.
I'm not saying you should give them up, but NYC has a lot of great clinics. RMA is supposed to be fantastic as well. I've never been to SIRM, but I am wary of their reproductive immunology treatment, b/c of the things Dr. Goldschlag told you. I've heard that from Stein and my current Dr. at Cornell - they both said that there is no scientific evidence to back up the testing/treatment they do. They believe they're taking advantage of people. Then again, it can't hurt to try those things, so I've thought about going there myself. A lot of girls have great experiences w/ SIRM.
At the least, I'd have a couple more consults...maybe RMA and SIRM...and maybe NYU. Maybe one of them would take your insurance.

FET - 2 blasts, 1 survived the thaw. Transfer 2/19. Beta #1 3/1 375, Beta #2 3/3 885, Beta #3 3/8 4261, Beta #4 3/11 9005. U/S 3/8 1 sac 1 yolk, U/S 3/16 1 heartbeat 114bpm!
James born Oct. 24th 2011 via c-section at 38 weeks!
Surprise BFP - Jack born April 28, 2013 via VBAC after PTL at 33 1/2 weeks!

i just want to say that i don't think patients are misled or being taken advantage off at sirm w/ regards to intralipids. many times patients that request the testing or the treatment have had several losses or never gotten pregnant with several attempts. i was never pushed to do the testing and still haven't had it done. we're adding it to my protocol b/c i asked about it and it only costs $50 and can't hurt in any way.
clearly it does help women get and stay pregnant (m&m is a success story). and each clinic has their own beliefs or way of doing things and some think outside the box more than others.
ultimately, go where you feel comfortable, trusting your RE is really important.
