My RE tends to use Long Lupron as his "standard" and utilized the other ones based on specific situations.
Married 08.06 Started TTC 05.08 Me: Stage II endo, borderline high FSH DH: perfect 1 lap, 5 IUIs = 4 BFNs and 1 c/p 2 IVFs, 2 FETs = 1 BFN, 1 c/p, 1 ectopic and finally a sticky BFP in May 2011!

This. Love AF and their great site. Dr. Sherbahn is also a LL devotee, but I think the protocol can also be driven by dx/predicted ovarian response.
TTK 9/06 / TTC 10/08 / Twins 12/11 / Life Blog 5 REs + 3 surgical hysteroscopies for septum/lap + 3 failed IUIs IVF w/ICSI/AH & acu = BFP!, unexplained spontaneous m/c @ 8w2d (our little girl), FET w/acu = BFP!, B/G twins!, lost MP @19w, dx w/funneling cervix @20w, twins nearly lost to IC @21w, saved by rescue cerclage, 17P & 16w of bedrest Our twins born @36w4d via CS when A came foot first
Re: What determines protocol?
There is some great information on that here:

1 FET in Aug 2013 = BFP!
Some REs have a preference, I am sure.
Antagonist is considered the easiest/quickest protocol.
Because I am DOR, I am doing Estrogen Priming Protocol which is part Antagonist.
There are even a few ladies that do protocols for IVF with Clomid (not as common).
Again, it is not a perfect science. One protocol may work great for one woman and not so great for another.
