We had better luck with flannel receiving blankets. Not the little rectangular ones like the Carters or Gerber ones, but larger, square ones that I made, that were about 40"x40". The Ultimate Swaddle Blanket is nice too (but the ones I made were very similar for a fraction of the cost!)
I totally sucked at swaddling, but DH was a pro. We had the best luck with large receiving blankets (like from the hospital), they were always tighter than any SwaddleMe blankets
I gave up on trying to swaddle the arms pretty quickly. I would swaddle right under the armpits and let his arms be free. we stopped swaddling around 2 months.

We also have best results with larger square flannel blankets. There are very descript directions in the book "Happiest Baby on the Block." They recommend tightening after each fold/wrap step. For me, the price of the book was worth learning to swaddle tightly (but all of the other tips have calmed our baby instantly and are also incredibly valuable).
I'd recommend either buying/borrowing the book or googling for videos or instructions.
With the SwaddleMe, make sure you are pulling it as tight as you possibly can. It seemed to me that each time I used it, it got a little more stretched out naturally, so the next time I used it I had to pull it tighter to fit DD in it.
I will say, though, that I had to quit swaddling around 8 weeks because she fought it so much and seemed much happier and slept better when she had some freedom to move her arms!

There is a Youtube video out there that will teach you how to double swaddle. It looks like a straight-jacket, but it looks VERY effective. Out little guy always wrestled out of his swaddles too.
We are big swaddleme ppl, and just stopped swaddling with them 2 months ago When she started wrestling an arm out, I just would swaddle her w/ an arm out and she seemed happier. I think the effort of getting an arm out would wake her up & make it less soothing! Will she sleep with one or both arms out?
I can't remember when we stopped using it. However, there came a point that DS didn't like his arms swaddled. So, as pp said, we just swaddled underneath his armpits.
