How carefully do you have to be with a preemie and other young children together?

I have a NICU preemie that has been home for a little over three weeks. I watch my friends 19 month old so she isn't in daycare. I keep the two away from each other, thanks to a nurse I hired. But they do come in contact in the car and sometimes while I am feeding the baby because I am without help for about 8 hours or so a day. What did you do to help prevent the preemie from getting sick? We have not allowed company over, and we wash and get the house and ourselves as clean as can be. As fall comes we will be even in a deeper hole because of flu season etc. I have had my friends baby in my care since she was 12 weeks old and she is like part of the family so she is here to stay. Just looking for answers from others who have a preemie and more than one child at home.
