SEVERE diaper rash - please help!!

We are just back from a week long beach vacation and my one son has the worst diaper rash I have ever seen.  He is red and puffy and there is even some blood.  I loaded him up with diaper creme but wanted to see if you had any other recommendations.

Would you call the pediatrician for a bad diaper rash? Should I be using something else besides diaper creme?

  • We battled a bad diaper rash over here a month ago.  If you see blood, I'd call your pedi to see if they want you to come in and get a perscription. 

    DS had blood with his and they ended up prescribing OTC items like Bactricin (sp) which is a topical ointment mixed with the purple label Desitin.  He also had mild yeast so we used an anti-fungal on him, too.  It took a full week to clear up but the mixture of those three items did the trick.  And his rash was really bad!

  • W/ open skin, call. They will most likely want to see him and prescribe a cream. My DD had it super bad too. The Rx they gave her was called (sp?) Nystasin. It really helped.

    dr. also said not to use wipes until rash cleared; use cotton pads and baby oil to wipe. He aslo said to make a paste of Mylanta and cornstarch--sounds crazy but it helped clear up the redness/irritation until the cream he prescribed kicked in.

  • With open skin, I always use Neosporin.  I'll not take our boys in for an open sore diaper rash.  They've had blood along with it countless times, but it's just bleeding while I'm wiping (I swear I wipe gently, I guess they have gentle tush skin) and it's always stopped after they first change.  When it's not bleeding, I've found Butt Paste to work the best.  I've tried several different creams, I tried the Mylanta trick, the only thing that works for any of my boys is Butt Paste and it works super quick IMO.
  • I have had this with both babies.  I used triple paste (expensive but amazing) on my little girl but then a friend told me the doctor told her to use nothing but petroleum jelly.  So when my son got an insane one after diarrhea diapers for two days (w/ a little bit of blood too), I used it and it worked!  amazing

  • Do you use pampers dry max? I've heard that the new dry max is causing bad diaper rashes.
  • One of my girls had a bleeding rash (only when wiped) around 1 month. New mom jitters sent me to the pedi, who prescribed a topical antibiotic ointment and recommended triple paste. It cleared up in about a week and a half. We used moist baby washcloths instead of wipes and tried to let her air out on a bunch of blankets a few times. I love the triple paste diaper rash creme. We use Dr. Smith's for "maintenance" but I think that triple paste will be my go-to when something comes up.
  • Thanks for all of the advice and suggestions.  I did call the pedi and they recommended that I start using wet cloth or paper towels instead of wipes.  And to let him air dryer as much as possible.

    They said to use Neosporin on the broken skin and lotrimin cream on the red, puffy skin.  And if it does not look better by Wednesday then I should bring him in.  I am also going to pick up that stuff you mentioned.

    They wear Huggies and Target diapers and never had a diaper rash longer than a day.  I think a week of eating sand was just too much for his little bum.

  • One of my girls had it really bad and we used the prescriptions mentioned above, but things weren't getting better. 

    What did help was we used Cetaphile (use your fingers to apply) and a dry cloth diaper to wipe her bottom at each change.  

    Then to dry it out, I started using my hair dryer on the cool setting (of course test on your hand first) to really dry her little behind.  Then I'd put the prescription ointment on, with Aquafore on top. 

    In two days it cleared up!  But we kept using the hair dryer bc she really liked it!  She would raise her little but in the air.  So cute!

  • We just went through this with one of my guys. Our pedi recommended a combination of Triple Paste, baby cornstarch, and Lotrimin. It took several days of vigilant diaper-changing and covering him with paste like frosting a cake, but it's pretty much cleared up now.
