My great grandpa's name was Antony. 100% Italian that relocated to the US in the 1920's. I love the name but I think if I used it now, people would just think its Anthony. Maybe as a middle name?

I have a friend named Tephanie.... pronounced TIFFANY, can you guess how often people add the S for her?
I actually PREFER Antony over Anthony. I wouldn't mix them up at all.

I think the safe bet is to middle name it.
(read: Marc Anthony and Jennifer Lopez- something people are more likely to know about these days)
It would be GREAT for a middle name with two syllables ending in a consonant. What do you have that fits that description?
